Case Coordination Unit - Sangamon, Logan, Mason
& Menard counties

The Case Coordination Unit provides a variety of services to individuals age 60 and older that are designed to assist senior citizens in remaining more independent.

The Information & Assistance (I & A) Specialist directs individuals and/or their family members to available services and community resources, which meet the individual’s expressed needs and can assist them with accessing needed services.

The Senior Healthcare Assistance Program (SHAP) Coordinator and the Medicare Part D Coordinators are responsible for Outreach and Information & Assistance activities related to the Senior Care Rx/State Pharmaceutical Assistance Program and Medicare Part D.

The Comprehensive Care Program (CCP) consists of trained care managers who specialize in a variety of programs that promote independence and prevent premature institutionalization. All care managers provide comprehensive assessments, at no cost, to individuals age 60 and older who are in need of long-term care options. Care managers identify needs and strengths of each individual and develop comprehensive care plans with the client’s direction. During the assessment, eligibility for the Comprehensive Care Program (Homemaker and Adult Daycare Service) and other programs are determined so individuals may make informed choices regarding their own care. Additionally, options are provided regarding long-term care facilities through the Choices for Care Program.

The Money Management Program (MMP) assists limited-income seniors with managing their finances. Insured and trained volunteers are appointed to provide a variety of services such as: assisting with opening/organizing mail, budgeting, checkbook balancing, bill paying and/or managing monthly social security benefits. The $enior Money Managers Program ($MM) provides similar services as the Money Management Program listed above. These include personal business assistance to customers who have difficulty in managing their personal monetary affairs. However, this program is geared towards middle income seniors and a sliding scale fee is charged for services.

The Elder Abuse and Neglect Program (EAN) responds to older persons who are victims of abuse, neglect and exploitation with the assistance of existing legal, medical and social service systems. Care managers work with seniors to develop and implement a care plan for the purpose of stabilizing the situation and reducing the risk of further harm.

The Crime Victim/Witness Assistance Program (CVWAP) provides assistance to individuals age 55 and older, who are victims or witnesses of a crime. This program is provided through a grant from the Office of the Attorney General. Program staff serves as an advocate for victims/witnesses and act as a liaison between law enforcement and other legal personnel.

Daily Bread - Sangamon & Menard counties

Daily Bread is a mid-day meal program offered at 12 congregate sites in Menard and Sangamon county. In addition to a nourishing meal, Daily Bread also offers participants an opportunity for fellowship, informative programs and social activities. Home-delivered meals are available for those individuals who are homebound and on a general diet. Arrangements for home delivery may be made through the Daily Bread program. To qualify for Daily Bread, participants must be at least 60 years of age; however, income is not a factor. The program has a voluntary contribution of $2.50 per meal to help supplement the partial grant funding. Meal locations include Athens, Auburn, Chatham, Divernon, Greenview, Illiopolis, Pawnee, Petersburg, Sherman/Williamsville (home-delivered only), Tallula and Temple Israel (congregate only), Washington's Cafe and the Springfield Senior Center in Springfield.

Senior Transport - Sangamon county

Senior Transport provides transportation to participants, age 60 or older, living in Sangamon County. Vans are wheelchair accessible. Policies are provided to participants with information regarding priorities and daily scheduling of appointments. A Personal Information Form (PIF) is required to be completed on each participant before service is provided. Transportation is provided to rural county participants. This service is offered on a weekly basis and encompasses the entire county area. Service for the greater Springfield area is offered Monday through Friday from 7:30 am-4:00 pm.

Elder Assistance Services (EAS) - Sangamon county

Elder Assistance Services is a program that provides social adjustment rehabilitation counseling through one-on-one home or office visits for advocacy. EAS provides assistance in dealing with physical, family or other related losses that occur as part of the aging process. Counselors provide assistance obtaining access to local, state and federal programs such as Medicaid, Food Stamps, Circuit Breaker and LIHEAP.
